SHIP Insurance Waiver

Signing the SHIP Insurance Waiver signifies that you are opting not to purchase the Student Health Insurance Plan (SHIP) as you already have a comprehensive private health insurance plan spanning the length of the IFLI Summer Program.

If you are currently insured, you must email the insurance card/policy that states your name. If you are on your parent’s plan and only their name is on the insurance card/policy, we need proof that the policy allows dependents

  • Sign the SHIP Insurance Waiver
  • By June 01, 2023, email proof of private insurance and SHIP Insurance Waiver to ifli@lpo.wisc.edu
